Program Name: Birth to Three
Connections
Services offered: Connections
offers families information about the development of their children
through developmental screening and evaluation. It gives support and
education to parents, provides early intervention services for the
eligible infant or toddler from birth to age 3, and links families
with other community resources. The UCP Dane Birth-to-Three program
serves families residing in Dane County school districts outside of
the Madison Area School District.

Hours of operation: 8:00a.m.
to 4:30p.m., Monday thru Friday. Different hours may be negotiated
to meet an individual family or child's need.
Size of staff: 18 regular staff
memebers and 30 community staff members.
Average cost of service is:
Evaluation, service coordination, and meetings to develop or review
the Individualized Family Service Plan are provided at no cost to
families. For eligible children, Wisconsin has a Parental Cost-Share
that is based on family size and income. The range of this annual
Cost-Share is $0 to $1,800 (capped).

Intake procedure is: The family
or, with the family's permission, anyone in the community may refer
by mail, phone, or email. After the program receives basic information,
an Intake Coordinator will make contact with the family to arrange
an intake - an informational meeting about the program, about parental
concerns and a developmental screening as appropriate. The parents
and Intake Coordinator will determine the next step (further information
about typical development at the child's age, other screening, or
evaluation at a subsequent home visit).
Target population is: Children
under the age of three about whom parents or others have concerns
related to developmental delays. The UCP Dane Birth-to-Three program
serves families residing in Dane County school districts outside of
the Madison Area School District.
Other eligibility requirements include:
Children may enter the Birth to 3 Program/ Connections in one of three
ways: having a diagnosed condition with more than 50% chance of resulting
in developmental delay(s); having a 25% delay in development of functional
motor, communication, cognition, social-emotional or self-help skills.
Additional information: Connections
is a voluntary, family-centered program in which parents are key team
members. The program is strength-based, working on developmental delays
by using the family and child's strengths and skills. It is responsive
to the concerns and priorities of the family. Connections adheres
to each profession's standard and Code of Ethics, the Birth to 3 Program
law (Wisconsin HFS 90) and its Guiding Principles. Connections also
reflects the mission and principles of its administering agency, United
Cerebral Palsy of Greater Dane County.
