Average cost of service is:
Cost depends on the child's needs and the county's ability to access
funding. There is a parental costshare for many of the programs.
Funded by: The county accesses
some federal and state funding as well as using local county levy.
Average length of service is:
Most people who are eligible for this service system will receive
long-term services.
Intake procedure is: Intake
for most children is through the Family Support and Resource Center.
Target population is: Eligible
children and adults as determined by Family Support and Resource Center
and Dane County.
Other eligibility requirements include:
The child must have a diagnosed developmental disability of autism,
mental retardation, seizure disorder, cerebral palsy, brain injury,
or Prader-Willi Syndrome. In addition, there must be substantial limitations
in 3 out of 5 areas of functioning: Self-care, Language, Learning,
Mobility and Self-direction that are directly related to the developmental
disability.
